Prepared Paper Work For My Disability Case Worker

When I was to my family doctor last I asked him to complete a medical travel form. This is for my disability to cover the expense of taking me to the health care facility where I am taking the course on living with chronic conditions, seated exercise program and will be taking the course on living with chronic pain this fall. If this is going to be helpful for me and I am to gain the maximum benefit possible I need to attend consistently.

This health care facility is only minutes from my home. The only time I need to hire someone to drive me there is as a result of inclement weather. Otherwise my power wheelchair is able to bring me there. It isn't my intention of hiring someone each time. I enjoy the independence my power wheelchair provides me with.

I've also received some updates about the disbursement of my dad's estate. I've been providing these to my disability case worker so he has a rough idea of what is happening with me. I care about having integrity. I've got this paper work in an envelope and will deliver it to the disability office when I want to get out of my home for a bit.
