Preparing For Medical Travel With The New Specialist

The disability pension I receive covers medical travel. After I arrived home from the appointment with my family doctor Friday afternoon I went to the disability office to get the medical travel form for my family doctor to complete.

I've just wrote a brief cover letter to my family doctor and prepared a self addressed stamped envelope for the form to be returned to me once it is completed. When I was with my family doctor we looked up the contact information of where this specialist has his office. I've provided these details in the cover letter to my family doctor so he won't have to look them up a second time. I happen to really enjoy doing the 'grunt' work for my care. I want to be involved in the process.
