Completed Physiotherapy Clinic Intake Forms

When I was to the physiotherapy clinic this morning to arrange my initial assessment I was given the intake forms to complete. I brought them home to complete.
I've just finished filling out their final form. Some of the questions asked I've provided indepth answers in the 6 page medical narrative I prepared early this morning.
I will deliver this to the physiotherapy clinic tomorrow morning. This will give the physiotherapist an opportunity to review their typical details in preparation to begin treating me.
