Dropped Off Summary Of My Physical Disability For My New Family Doctor Intake Appointment Tomorrow

I've been thinking about how to make the transition to a new family doctor as easy as possible for both myself and the new doctor office. I prepared a 3 page summary of my physical disability to provide an overview and to help the medical team understand me and ask helpful questions to me. What I am talking about isn't 3 pages of ongoing writing. These are charts:
- A list of my orthopedic surgeries and their intended purpose
- A summary of my pain management regime
- A summarized medical history of my knee joints
I've delivered this to the doctor's office. I'm now ready for my nurse intake appointment tomorrow.
