Requesting Copy Of My August 4th 2017 Bone Scan Report

Logistically speaking it will be easiest for me to obtain a copy of my bone scan report from the surgeons office who will be assessing me at the end of October. This report is needed for a health care professional to be able to justify the recommendation for an upgraded mattress for my electric "hospital" bed. I've left a phone message explaining why I am asking for this. The surgeons administrative assistant is great at returning phone messages. As I live alone my voice mail is private and confidential. It is okay to leave me a message when I am resting or doing my chores.
