Inquiring About My Disability Medical Transportation Coverage

I have some follow up to take care of following my medical appointment yesterday. I need to get a medical travel form completed so my disability benefits will cover the expense of the para transit vehicle to take me for blood work and physiotherapy. I've looked up the addresses and phone numbers of the facilities where I need to attend, printed these off and prepared an envelope for mailing so the form may be returned to my disability case worker. When I have my next social worker appointment I will leave this at at the doctor office to be completed.

The next natural step is the working out of the transportation itself. I have just e-mailed the not for profit organization scheduler I normally deal with to inquire about the practical logistics of this and their general availability. She has been wonderful to me. When I first started using their services they asked me to work with them and in turn they'd work with me. They provide car pooling. There is often more than 1 person being on the para transit vehicle at one time. I need to need what times(s) work well for them with predictable travel. I try hard to hold up my end of the agreement. Then I will book the blood draw appointment.
