Disability Medical Travel Form Delivery: Take 2

I am appalled at what has happened. I sent a medical travel form for my family doctor to complete. It was planned that I'd have a virtual appointment for this to be filled out. I included a stamped and addressed envelope for my family doctor office to mail the completed form directly to the appropriate disability case worker. Amazingly the envelope was opened and then refused delivery.

It has just been returned to my through Canada Post. I just got off the phone with my disability case worker. He has asked me to scan the form and submit it electronically. I will need to make a separate complaint against the mail room personal for this shenanigan. I had the name of my disability case worker on the envelope. The form itself had my member ID on it.
