Requesting A New Medical Travel Form

I've just left a voice mail message for my disability case worker. I've requested a medical travel form. My disability pension pays for travel to the appointments I have. This form will be to validate the travel to the sports injury doctor after the MRI scans of my knee joints is completed. I will have my family doctor complete this form when I have my next appointment.
There is no urgency in having this form completed sooner. The involvement of a sports injury doctor won't happen until the summer of 2017 at the earliest. I've long since learned if there is a task relating to my health care that I should complete this when I have the energy. Otherwise I might not get it done. In practical terms I enjoy doing the grunt work for my health care. I want to be involved and not passive. If there is something I am able to do to help myself I want to do so. My family doctor and I make a great team.
