Requested Medical Travel Form For My Disability Pension To Cover Travel To The Dentist

I just left a phone message for my disability case worker. I've requested the form for my family doctor to complete that authorizes medical travel coverage for my dental appointments. The city where I live has made the downtown all fancy. Interlocking bricks have been installed in the road and sidewalk. The suspension system for my power wheelchair transits the vibrations to my body. I can't take the pain. It is preventing me from taking care of myself for the following day and a half. Going forward I need a para transit vehicle to bring me to my dental appointments.
