Mailed My Disability Tax Certificate Renewal To Canada Revenue Agency

A few weeks ago my family doctor completed the Canada Revenue Agency form for me to renew my disability tax certificate. Only we missed the part to check the designation of the health care professional completing the form. While I was at my appointment to complete the dietary special allowance for my high blood pressure I had him check the appropriate spot. Since this is done I have mailed the completed form to Canada Revenue Agency for processing. I can cross this off my "to do" list.
