Dietary Special Allowance For High Blood Pressure

I just finished the appointment with my family doctor for completing the Ontario Disability Support Program form for me to apply for the dietary special allowance for high blood pressure. I really like my family doctor. I knew the appointment would go well. But it is more than this. He has been very supportive of the effort I am putting into my life and systematically addressing the issues that are cropping up. I really appreciate this. It is really nice to have a family doctor that is supportive of me and doing what he can to help me improve my quality of life. I am heading over to use the warm water therapy pool now. Although I have a few chores to do on the way there. When I get home I will scan this and submit it electronically for my disability case worker to review.
