Letter To Family Doctor With Pain Control Medication Request

I wrote a short letter summarizing my pain control. I just had the pharmacy I use fax this with new pain medication prescription requests to the family doctor that is retiring. This is one of the things that needed to be in place for my switch of family doctor in the coming days while my physical disability is assessed.
