Completed Intake Form For New Family Doctor

I have just finished completing the intake form for my new family doctor. My appointment is tomorrow. Some of the questions are what I already covered in preparing for the appointment. I already provided a list of all the surgeries I have undergone, my medication regime, recent colonoscopy, gastroscopy and fecal analysis because of the diarrhea. Over overall themes of the intake form were to establish a medical history and preventative medicine strategies. It was highly detail oriented. I don't mind this type of thing. But I am mentally tired now from being in pain. The good news is that it is done. I will be able to talk this over with my new family doctor tomorrow. I genuinely hope I have a good night rest so I am able to easily recall information relevant to tomorrow's appointment.
