Picked Up Medical Travel Reimbursement Form

I've just been to the disability office. I picked up a medical travel reimbursement form for my family doctor to complete. The specialist I am being sent to has his office about 75 minutes from my home. My disability pension covers medical related travel. I am getting the mechanism in place so when an appointment is available I am able to accept and attend it.
